Structure Of Report Writing

Does this sound good for a sample report?

The school I want to work at has asked me to write a sample report. I have never been a form tutor only a subject teacher so I need to know what you think of this sample I wrote. Any help is appreciated.

Anna is a delightful member of the form that needs to learn to occupy her time constructively and is developing a better attitude which she must continue throughout 11th grade if she wishes to succeed in the final examinations.
Anna has a good attitude towards school and has taken part in school events. She is gaining independence - is too easily distracted but is an energetic participant in class activities and is a positive member of the class. She is most successful with consistent structure and monitoring by her teacher. Anna will experience increased success as she develops a sense of personal responsibility toward her learning. It is advised to work some more on her physics and mathematics over the Christmas break so that she can maintain a good level.

In one sentence you say she must develop a better attitude and in the next sentence you say she has a good attitude.

I would do it this way:

Anna is a delightful member of the form. She does need to learn to occupy her time constructively as she wastes some time in class. (Explain how she isn't using her time constructively.)

Anna has a good attitude towards school and has taken part in school events (such as ....). She is gaining independence (what does that mean?? Explain.) However, she is very easily distracted in class (how could this be remedied??) Anna participates energetically in class activities and is a positive influence in the class. (How??)

Anna does her best work when the class is very structured and when she is closely monitored by the teacher. (You might link this sentence with the one about using her time constructively.)

Anna will experience increased success as she develops a sense of personal responsibility toward her learning. It might be a good idea if she worked a little on her physics and mathematics over the Christmas break so that she can maintain good grades in these subjects. (However, why would you suggest that she work on these over the break if she already has good grades in these areas??? Do you really mean to say that her grades in these areas could be improved??)

You haven't mentioned whether you think she is working hard. How about homework assignments? How are they completed?

Good luck with it.
