How do i do a table of contents for my report?

I have a dance assignment on the history of ballet and we have to write it up as a full report, even though its not an experiment or anything. How should i write up my table of contents? I have two main sections, the general history and specific techniques, consisting of several paragraphs each, an introduction and conclusion. It's fairly short, but any advice on how i could format and label the contents page would be helpful =]

Also, how should i go about writing a recommendation if i have only been researching and not experimenting?
