How to write up a chemistry lab report?

My lab report has to include the answers of some pre-lab and discussion questions.

Should I write up each question followed by the answer?

Or should I paraphrash the question into the answer and just make it look like an essay?

Any help would be much appreciated. :)

i usually use this outline:

AIM
INTRODUCE THE TOPIC
PROCEDURE (inclusde apparatus etc)
RESULTS
DISCUSSION (answer any questions and discuss results)
CONCLUSION
