equine assisted therapy?

hello everyone.
i have just started my own riding therapy company where i live and it's running very well.
after doing all my training in germany it's great to finally be able to work. the only thing which is causing me a bit of a headache is i need to write progress reports.
now i know what i should put in it but i am not sure how to go about it.
i would love to just have a look at some of your forms or reports you use as riding therapists as well to get a better overview, what has to be in, what i might not even have thought of. if you could show me a template or so i would be very grateful.

If you are a certified Hippotherapy center, they will give you guidelines by which to conform. If not, just google your nearest organization and they will provide not only the guidelines but provide the forms by which you can complete and be in compliance with their rules and regulations. I just want to say that it does take a special kind of individual to offer this service and I commend you for it.
